Team Certificate/Prize Form

Certificates
Information on this sheet is necessary to receive personalized certificates of participation. K -12th grade team members and the advisor listed on this sheet will receive a certificate of participation that will be mailed out at the end of the semester. If certificates are needed by a certain date, please note in comments section or email that request to kcee@wichita.edu.

Prizes
K -12 student teams with a portfolio value in the top five percent in their grade region at the end of the session will also receive prizes. This year's prize will be SMG T-Shirts. This form must be filled out to receive the prizes. You will be contacted if your team qualifies for t-shirt sizes. Click here to view T-Shirts

Participant names will not be used for any purpose other than printing their names of certificates. If you do not wish to list each individual name, just note that you would like blank certificates in the first name field and be sure to include the number you are requesting.

Do not fill out until your team participants have been determined and you have received your team ids & passwords.
